Mikkelsen was born in the Østerbro area of Copenhagen, the son of Bente Christiansen and Henning Mikkelsen, a cab driver. He is the brother of actor Lars Mikkelsen. After attending Århus Theatre School, he made his film debut in the movie Pusher.
